By using the Zaptag service having instant access to your health records has never been easier. Zaptag provides the means to store information from many different sources.You can add information in 2 ways:

Yourself - Using our Health Wizard you can enter your known medical information, such as Allergies, Medication, Conditions, Next of Kin. |

Your Doctor - Your health professionals can now simply add a copy of your medical information to your account. You will need to complete a consent form and present it together with our Doctor's guide to your health provider. The consent form and the Doctor Guide can be downloaded below. You will receive an email notification when you have been sent any new records. |
