Medical Card Terms and Conditions of Use
ATTENTION: PLEASE READ THIS DOCUMENT CAREFULLY BEFORE
PURCHASING YOUR ZAPTAG MEDICAL CARD.
The Zaptag Medical Card and the accompanying
documentation are provided to the End-User by Zaptag Limited
("Zaptag") for use only under the following terms:
- The End-User assumes sole responsibility for the
installation, use and results obtained from use of the
Zaptag Medical Card.
- Data stored on the Zaptag Card is safe in the
following environments:- X-Ray security systems at airports.
- For End-Users in the USA, changes or
modifications made to this equipment not expressly approved
by Zaptag may void the FCC authorization to operate this
equipment. For End users in Europe, this product conforms to
CE directives.
- The Zaptag Medical Card may not function
correctly if it is connected to a computer when the computer
is started up, restarted or restored from sleep mode. Always
disconnect the Zaptag Medical Card from your computer before
performing any of these operations. Do not place the Zaptag
Medical Card in water, any locations that are extremely hot
or cold, dusty or dirty, very humid or where it is exposed
to corrosive gases.
- The End-User may only use the Zaptag Medical
Card strictly for the holding of personal health records and
no other information may be placed on the Zaptag Medical
Card.
- The End-User must register the Zaptag Medical
Card by logging on to www.zaptag.com or by telephoning
Zaptag on 08456 120510. In the course of the registration
process for the Zaptag Medical Card the End-User will be
required to provide certain personal information. This
personal information is subject to the Zaptag Privacy Policy
which can be viewed at www.zaptag.com; alternatively please
telephone Zaptag on 08456 120510 and we will send you a copy
of our Privacy Policy.
- The Zaptag Medical Card is protected by
copyright law and is the subject of other intellectual
property rights that are owned or licensed to Zaptag.
- The End-User may not decompile, reverse
engineer, disassemble, or otherwise attempt to modify the
Zaptag Medical Card. The End-User’s personal health
information that is held on the Zaptag Medical Card is
password protected. Zaptag neither backs this information
up, nor does it hold duplicate information. In the event
that the End-User loses the Zaptag Medical Card or loses
their password, Zaptag cannot retrieve information held on
the Zaptag Medical Card. The End-User has sole
responsibility for safeguarding the security of their Zaptag
Medical Card and password.
- Zaptag’s product warranty over the Zaptag
Medical Card is limited to the Zaptag Medical Card itself
when used normally in accordance with the Operating
Instructions. Any services provided by Zaptag are also
subject to these restrictions. Zaptag cannot accept
liability for damage or loss resulting from the use of the
Zaptag Medical Card, or for any claim of whatever nature
from a third party. Zaptag cannot accept any liability for
problems with your computer or other hardware arising from
the use of the Zaptag Medical Card, the suitability of the
Zaptag Medical Card for particular hardware, software or
peripherals, operating conflicts with other installed
software, data loss or other accidental or unavoidable
damage. Zaptag cannot accept any liability for financial
damages, loss of profits, or any claims from third parties
arising from the use of the Zaptag Medical Card.
- General.
These terms are governed by and construed in
accordance with the laws of the England and shall be subject
to the exclusive jurisdiction of the courts of England and
Wales. If any provision of these terms is held by a court of
competent jurisdiction to be invalid or unenforceable to any
extent under applicable law, that provision will be enforced
to the maximum extent permissible, and the remaining
provisions of these terms will remain in full force and
effect.
